Stainless Steel Pet Tags (Slide-On)

Pros: Nice stainless, not super-easy to bend, good size, price
Cons: Laser etched (not engraved), flat (but can be shaped with a soft mallet).

If your dog wears this tag daily then durability might be an issue, but overall a good buy. The description also says "back", but there's no customization option for printing on the back of the tag.
